I informed him that the talisman was with the priest, and he agreed to take me up to see Thomas.

After talking to Thomas's sister, I learned that the medicine is located in another tower. I headed over and entered the kitchen at the base of the tower (it seems to be the kitchen). A guard mentioned he was hungry, so I gave him some food I had picked up, and he left. I then made my way up to the alchemy room where the medicine was seemingly in a nearby chest. Thanks to my blacksmith skill, I was able to forge a lockpick after making horseshoes earlier (PS: It seems I could freely roam Thomas's tower, so I looted it clean, carrying over 330 weight without issue.)

After retrieving the medicine, I returned to Thomas, which triggers an important scene in the game. It’s that simple!
