Step 1: Locate Captain Thomas
Captain Thomas is located on the upper floor of the Trotsky Castle (at the top of the Maiden's Tower), where he lies feverish on his bed. You can distract the guards at the stairway by throwing stones, and then sneak up. To access the east side of the castle, you first need to help the blacksmith craft horseshoes.
Step 2: Create Fever Medicine to Heal Captain Thomas
To treat Captain Thomas effectively, you will need to gather the required ingredients and craft the fever medicine at your alchemy station. This medicine is essential for reducing his fever and ensuring he survives.
How to Heal Captain Thomas in Kingdom Come: Deliverance II
To start, head to the top of the west side of Trosky Castle (Old Lady Tower). There will be guards in the kitchen. You can find food from the bathhouse directly beneath the kitchen to give to the guards, making them leave. Near the top floor (going down two levels from the top), you can find an alchemy table. On the left shelf in the room with the alchemy table, you will find the Doctor's Journal, which contains the recipe for the fever-reducing potion. Then, pry open the chest in the room to get the alchemy materials. A lockpick can be purchased from the stable worker, Kabat, that you previously helped, but it’s quite expensive. You can get it for free by retrieving the turquoise beads from the priest for him (you can either steal them or knock out the priest to take them).
Then, use the alchemy table to brew the fever-reducing potion. The recipe is shown in the image below.
