- Start by honestly moving all six bags, then find the blacksmith to take on the task of making horseshoes and get permission to enter the inner courtyard.
- After making the horseshoes, return to the blacksmith to turn in the task and hand over the horseshoes to the groom. If you selected the perk to recycle scrap iron into a lockpick while crafting, you can skip the next step.
- If you have some money, look for the groom to buy lockpicking tools. If you don't have money, play dice with the priest to win back the items from the groom and trade for the lockpicking tools.
- Speak with the female cook in the inner courtyard and help her pick the lock on her chest to gain access to the kitchen under the lady's tower.
- In the kitchen, you will find the steward who has a stomach ache. Convince him to let you prepare medicine, granting you access to the old woman's tower apothecary.
- The old woman's tower is on the right side of the entrance to the inner courtyard. Follow the stairs guarded by soldiers all the way up, and enter the apothecary. Find the doctor's diary on the bookshelf, read it to acquire the digestive medicine formula, then pick the lock on the chest to get the herbs and prepare the medicine for the steward.
- Return to give the medicine to the steward, and you will be granted access to the lady's tower. The lady's tower is directly above the kitchen where the steward is located; simply go straight up.
- Upon reaching the tower, you will meet the captain's sister and trigger the antipyretic quest. Return to create the antipyretic and submit it to complete the mission.
